Modern orthodontic practice does not only aim to straighten the crowed or crooked teeth, but more importantly to restore an esthetic appeal of soft tissue and establish a functional occlusion.

Facial Esthetics

The primary goal of treatment becomes soft tissue relationships and adaptations, not Angle’s ideal occlusion. Soft tissue relationships, both the proportions of the soft tissue integument of the face and the relationship of the dentition to the lips and face, are the major determinants of facial appearance. Soft tissue adaptations to the position of the teeth help determine the orthodontic plan and whether the orthodontic result will be stable.

For example, the patient in the picture below originally had crowded teeth with excessive incisor protrusion, upper and lower lips could not close in the rest position. Theoretically, both premolar extraction and palate expansion can solve this problem. In this case premolar extraction is the one that should be chosen because palate expansion cannot help with the closure of lips, while extraction can help retract the incisors, thus lips can close, improving facial esthetics.

Functional Occlusion

Temporomandibular (TM) dysfunction, to the extent that it relates to the dental occlusion, is best thought of as the result of injury to the soft tissues around the TM joint caused by clenching and grinding of the teeth. Given that, an important goal of treatment is to arrange the occlusion to minimize the chance of injury. A stable occlusion can also prevent relapse in the future.

Practice Based on Evidence

The era of expert-based orthodontic practice is over, and now we are entering into the era of evidence-based practice. Evidence-based orthodontics uses the relevant literature to determine the benefits and risks of alternative patient management strategies and to weigh those benefits and risks in the context of the individual patient’s predicament, helping dentists know the degree of uncertainty for particular clinical decisions. Practicing evidence-based orthodontics requires dentists with strong research and reading ability. Practitioners should know how to frame a clinical quandary to facilitate use of the latest English articles in its resolution, to evaluate the strength of the methods of the studies and to extract the clinical message, apply it back to the patient, and store it for retrieval when encountering similar cases in the future.

For example, with the popularity of invisible orthodontic practice, enamalplasty is being used more and more often. Whether enamalplasty could cause decay was always controversial. But, according to evidence-based view, the clinical results demonstrate that enamalplasty does not increase the risk of developing cavities. Therefore, enamalplasty with careful polishing is an optional method to solve teeth crowding problems.

The Aim of Implantation is to Restore Missing Teeth

 An implant with prosthesis is consisted of an implant, abutment post and crown. The implant is placed in the jawbone to serve as an anchor for a crown. The crown plays a major role for chewing. The abutment post is a component that connects the implant and crown. A dental implant is the best way to restore a missing tooth, for its structure mimics a natural tooth.

Procedures Involved are Carefully Guided

 The position of an implant should be determined by that of the crown above; this is what is referred to as “implantation guided by prosthesis,” in the International Implantation Guide. Without critical analysis of implant position before surgery, or use of surgical guide for implant placement especially when several teeth are missing from the adjacent area, the implant position may not be ideal, so it may have a negative influence on the implant during chewing, further causing the failure of implantation.

40μm Fine Bite Adjustment

 The complexity and demand for precision of the crown above the implant is higher than normal crowns, because the natural root has about 40μm of physical movement inside the alveolar bone, whereas implants bond with the bone tissue to obtain stability, which allows bare movement. Therefore, the 40um fine bite adjustment for the crown is also a key procedure for a successful surgery.

Implantation requires dentists’ substantial knowledge and clinical experience, and keeping up with the latest international progress as well. “subtle operation,” “painless”, “only a minor reaction after surgery”, and “the implant feels like a natural tooth,” are constant feedback we hear from our patients.

Rubber Dams are Used Throughout RCT

 The rubber dam serves two main functions:
        ① Isolation
        The tooth or teeth to receive treatment are isolated and exposed for the procedure, presenting better viewing and access.
        ② Barrier
        The dam material protects against infection, thus increasing the success rate of RCT; it can also protect adjacent tissues from accidental injuries from caustic materials.

Delicate Pulpal Wall Planing&Smoothing

 ① RCT is conducted with enlarged vision of working area and improved LED lightening, thus chances of canal missing or files being broken inside canals can be reduced to the minimum.
        ② Insufficient pulpal wall planing and smoothing can’t remove all bacteria effectively, resulting in the failure of RCT, whereas excessive pulpal wall planing and smoothing can decrease the fracture resistance of roots. Moderate debridement plays an important role in long-term effect.
        Both proper equipment and rich experience in RCT work the best.

Advanced Warm Gutta-Percha Obturation Technique

 Warm gutta-percha obturation is the standard international root canal filling technique. It has better sealability than cold lateral condensation, and even the fine canals can be filled properly. This technique guarantees the high success rate of RCT.
